Services Covered under the Warranty Policy The warranty policy is established to ensure service quality and the best interests for our valued customers when using services at Thuan Kieu Dental Clinic.

Conditions for Application Customers are eligible for warranty coverage when meeting all of the following conditions:

1. Valid Service Usage:

   • Used services or products directly provided by Thuan Kieu Dental Clinic.

   • The warranty request is submitted within the specified warranty period for each service type.

   • Warranty services are performed exclusively at Thuan Kieu Dental Clinic.

2. Presentation of a Valid Warranty Code:

   • Present a valid electronic warranty code or active warranty card/voucher.

   • The code must match the treatment records and history in the clinic's system.

3. Compliance with Care Instructions:

   • Maintain proper home oral hygiene and care as instructed by the dentist.

4. Regular Check-ups:

   • Attend follow-up visits, undergo check-up X-rays (if required), and get dental cleanings at least once every 6 months according to the clinic's appointment schedule.

Scope of Application The warranty policy is applied uniformly across the entire Thuan Kieu Dental Clinic system, allowing you convenient access for check-ups or warranty services at any branch.

Warranty Exclusions Customers are not eligible for warranty coverage under the following circumstances:

1. Non-compliance with Instructions:

   • Skipping follow-up appointments, improper oral hygiene, frequent smoking, failing to take prescribed medications, or not following the dentist's instructions.

2. New Medical Conditions/Health Factors:

   • Teeth or gums affected by new medical conditions such as diabetes, radiation therapy, or other systemic health issues.

3. External Impacts:

   • Fracture, dislodgement, or damage caused by accidents, impacts, sports activities, etc.

4. Natural Aging:

   • Changes resulting from age or natural degradation of tooth and gum tissue over time.

5. Treatment Outside Thuan Kieu Dental Clinic:

   • Dental work performed at another facility prior to treatment.

   • Interventions made by another dentist before returning to Thuan Kieu Dental Clinic.

6. Changes in Requirements or Accepted Risks:

   • Requesting modifications to results that differ from the original treatment plan.

   • Proceeding with treatment despite dentist warnings regarding risks of chipping, breakage, or poor durability.
